Create an Account from an Invitation:
If you are looking to create your FollowMyHealth account from an e-mail invitation, please follow the step below:
Click on the registration link from your email. You will then see the following screen with the below option :
By default, the email address where your invitation was sent will be in the username field. Enter the Invite Code: Last four of social or year born
- This is the invite code entry. It can be found in the invitation you received from your medical organization.
- Input the Invite Code in the box
Only records available office notes only. Testing results will not show on this platform
You must receive an email from Winchester Neurological Consultants to connect to your FollowMyHealth account.
